背景
11號避難所應該和其他避難所一樣都在2060-2070年之間蓋成的，跟大部分避難所一樣有 社會實驗的項目。實驗中，11號避難所的居民被告知他們每年都要犧牲一名居民，否則避 難所將會殺死所有人。不過事實上，如果居民拒絕的話，避難所會播放「自動解決回應」 ，這段訊息會稱讚這種行為是「人性光輝的範例」，沒有人會因此被殺。訊息還會告知大 門會立刻解鎖，跟監督確認外頭安全後便可出去。不過當11號避難所居民拒絕繼續犧牲時 ，只剩下五個人了。
「選舉」和監督訂的法規
從電腦終端上收集到的證據可知，由於避難所每年要犧牲一個人，因此在監督任期結束時 ，他需要前往監督室底下的一間房間並被避難所電腦殺掉。電腦並沒有指定死的人一定要 是監督，但在最早住入避難所的居民中只有監督知道死一人或全死的規定。在被告知規定 後，已經被封死在避難所裡的居民在震驚與憤怒中，選擇（被他們視為背叛者的）第一任 監督當地一位犧牲者，這個決定使的監督和犧牲者的位置合為一體，直到最後。

玩家在避難所大門發現的第一卷錄音帶記錄了這五人的命運。這五人以自殺的心情反抗避 難所電腦並拒絕做出任何犧牲，最後卻發現他們的反抗行為正是電腦想看到的，電腦讓他 們離開避難所並恭賀他們的無私行為。

In 2281 the reactor and the water purification system of Vault 11 has been damaged by age and the destruction from a civil war among the dwellers. Radioactivity, and especially radioactive water, are some of the hazards of the vault. It is therefore advisable to pack some RadAway and Rad-X when journeying to Vault 11. Since portions of the vault are under water, you may want to head over to Nellis Air Force Base and acquire the rebreather (from the quest, Volare!) that allows you to breathe under water before entering this vault, although there are also air pockets in the flooded areas. Other than the radiation on the lower levels, the only enemies you will encounter inside the vault are giant mantises, giant rats and two bark scorpions just inside the entrance from the Mojave Wasteland.

The stairs nearest the entrance hall lead to a floor containing a lab, and more stairs down. The stairs on the opposite end lead to a floor containing a computer room. One of the computers is booby-trapped, and will explode if you try to access it.
The entire vault has been plastered with propaganda for an election for a new Overseer, but each candidate is expressing outrage at having been nominated and trying to point out not only their innocence but why the other candidates deserve to be Overseer for their sins. Deeper in the vault, sandbag barricades, multiple traps, and several skeletons indicate that there had been prolonged fighting at some point.
居住區
順著樓梯下去會到達一個大房間，左右兩側是男女宿舍區，剩下的那個門通往警衛室和下層。宿舍區是由一系列互相連通的單間組成，也是唯一能在衣櫃里找到11 號避難所服裝的地方。在這裡有一台上鎖的電腦終端，破解以後能聽到另一份聲音記錄，而密碼就在幾步之外的一個柜子里。
在女宿舍區可以找到一本拳擊畫報，它在右手第二間房內，那裡還有一個隱身小子。在男宿舍區的某個房間內，一個很大的拖把桶後面藏了一些火焰噴射器燃料和5.56毫米子彈。
警衛室里有不少彈藥，但是沒武器，走廊內另外還有一個門，裡面有台電腦，不過都上了級別很高的鎖，在那房間內可以找到迷你核彈。在與警衛室相反方向的樓梯最下端有道門，通往11號避難所下層建築。
底層
下層有一條長長的過道，監督辦公室就在過道盡頭，在房間里有個上鎖的電腦和一些四散的武器彈藥。通往中庭的主入口被堵死了。
在鋼鐵兄弟會的任務「Still in the Dark」中，需要一個任務物品，叫做「差分壓力控制器」。它就在那條被輻射的水浸淹的走廊中間的房間里的一個柜子里（具體位置是在你必須撬開一個中等難度的門鎖並且游過被完全淹沒的反應堆區之前的地方）。
所以玩家必須下樓梯穿過發動機房，那裡被水淹沒並且充滿了輻射。因此推薦玩家在進入11號避難所之前多帶一些Rad-Away和Rad-X以備不時之需。在發動機房只有一個地方可以換氣，所以玩家必須小心，以免溺水（推薦打開一兩道門以後就折回到換氣點換氣，之後再開一兩道門你就可以見到通往上層的樓梯了）。
之後就可以進入中庭了，所長辦公室內的電腦密碼也可以在中庭二樓走廊上的電腦終端上查詢到。在中庭的演講台上有一份演講稿的磁帶，裡面也有密碼。在下層建築中還有咖啡廳，那裡有不少吃的，注意咖啡廳進門的地板上有不少機關，不小心就會要了你的命。在咖啡廳對門你會看到一個廚房，但是那裡有煤氣泄漏，並且有個炸彈陷阱，廚房門口就是機關，如果你不慎觸發了，那就會引起連鎖反應。
犧牲室
在抬腳進入所長室之前請小心，這裡有個絆線陷阱。利用之前得到的密碼登陸所長電腦可以開啟犧牲室。它被隱藏在椅子下。進入以後會是一條長長的，保存完好的走廊，與避難所其餘銹跡斑斑的地方形成了鮮明對比。在這裡遊戲會自動存檔（不是一般的那種自動存檔，而是一個另外的存檔），一個事先錄好的聲音會在玩家接近一個充滿陽光，而實際只是四盞三角架聚光燈發光的房間時響起，給玩家進行解說和指示。在這間房間裡面有另外一道門開啟，通向一個中間擺放了一把椅子，還有一台投影儀的房間。在影片完結時，牆壁會消失，牆上的隱藏門會打開，一群機器人和自動機槍會殺掉玩家。如果你在影片放完的時候還傻坐在椅子上的話，那你多半會被打成馬蜂窩。如果玩家先前在34號避難所（Vault 34）的軍火庫內拿到了脈衝槍（Pulse Gun）的話，那麼這場戰鬥會非常簡單，一槍就可以崩了它們。或者你可以戴上隱身小子，確保不碰到任何機器人和機槍塔，蹲下來偷偷溜到旁邊的房間，登錄主機解開門鎖下載資料後逃命。
