Overseer Barstow next complains that the Hippocratic Oath is backwards and that doctors should focus on the greater good of the Vault, rather than patients' welfare. She requests you to build a phoropter prototype, which must be powered and programmed as per the previous objects.
The three parameters involved are Subliminal Messaging, Vault Monitoring, and Improved Eye Care. Most companions will hate the negative parameters and like the positive one.
There are a few possible outcomes for this quest:
- Choosing all three positive parameters will result in the unnamed settler expressing satisfaction and staying in the Vault regardless, but Overseer Barstow on the other hand will feel disappointed.
- Choosing only one negative parameter will result in the unnamed settler questioning the experiment and attempting to leave; a speech check can be used convince them to stay.
- Choosing two or more negative parameters for the three projects will result in the the unnamed settler getting angry and leaving the Vault.
After completing the quest, the phoropter can be mass produced. It's located in the Vault tab under Resources, while in workshop mode. The stats of the device depend on the experiment option that was chosen.
- Selecting the subliminal messaging experiment option makes the device provide 15 points of happiness for 6 units of power, at the risk of killing a random non-essential settler (2% chance per day.)
- The Vault monitoring option provides 5 points of happiness for 3 units of power, and can also kill settler (1% chance per day.)
- The improved eye care option provides 10 points of happiness for 3 units of power, without any risk of death.

Notes[]
- If you can't speak to the overseer or your subject during any stage, check to ensure the experiment item is powered, and not sitting close to anything that will prevent the subjects from using the item. If the area becomes too cluttered, you may not be able to start the test and thus this interferes with the progression that allows you to speak to anyone (or even enter Workshop mode).
- If the conversation progresses to the point that Clem tells the subject to have a seat, but the subject fails to path to the prototype, using the setpos x and setpos y commands to directly move the subject in front of the phoropter will usually allow them to find a proper path and allow the quest to continue.
- If the overseer states the device isn't working even though all requirements have been met you can use setstage xx004F72 400 to advance the quest to the next stage and talk to the settler on the results of the experiments.
