Vault reactor incident and vault reactor incident (edited) are two variants of the same holotape in the Fallout 76 update Wastelanders.
Locations[]
- The first tape is obtained during the quest Overseer, Overseen, from the vault security terminal at Vault-Tec University, if the player character agrees to Head Engineer Brass' plan.
- The second, edited tape, is obtained during the same quest with the same decision by the player character.

Head Engineer Brass: Don't worry Kelemen! We're getting you out of there!
Doctor Loris: Are you insane? The entire reactor is flooded with radiation. We need to quarantine this whole area.
Head Engineer Brass: We're not leaving him in there!
Doctor Loris: It's not about him anymore. He's already dead. Anyone we send to help is just going to die with him.
Head Engineer Brass: You don't know that!
Doctor Loris: You're emotional. Not thinking rationally. Medical is locking this area down. That's all there is to it.
