A two-headed bear is the symbol of the New California Republic.
The two-headed bear is presented exclusively on the flag of the New California Republic, which is aped from the state flag of California. A two-headed bear has never been depicted in the Fallout universe outside of the NCR flag. Given that the state flag depicts a California grizzly bear, which went extinct in the 1920s with only sporadic sightings in the following decade. It is unlikely such a bear actually exists and can be seen as a metaphor for the NCR's founding in a post-War wasteland, home to a variety of mutated life forms, including the two-headed brahmin.
The only mutated bears that are actually seen in the Fallout universe are yao guai, which only have a single head.
