The twisted muscles serum is a consumable item in Fallout 76.

A flask of green serum which, when consumed, will cause the player character to develop the twisted muscles mutation. This mutation will increase the player character's melee damage by 25% and also improve their chance of crippling limbs. However, it will reduce their gun accuracy by 50%.
