The note can be found underneath Quentino Lombardi's corpse on the bed in the guesthouse behind his office, found in the Neapolitan Casino in Atlantic City during Regent of the Dead.

Tony,
You're probably never going to read this, are you? It's the most hopeless feeling in the world, I tell you, just scribbling into the void while I can practically feel my body giving in from the inside out knowing that you'll never even lay eyes on this piece of paper.
I know you feel like you let me down, but the truth is, I let you down. I was supposed to watch your back, keep an eye out for you. I lost track of things, and it all started spiralling out of control. If I could turn back time, I would've burned that first shipment of Blood. But we both know I can't do that.
It's poisonous, the damn thing. It's like the sweetness of sugar coursing through your blood. But just like how sugar rots teeth, there's something about this chem that decays your blood, makes it ache. Everything that flows through me aches.
I should've been alive to hand the reins of la cosa nostra to you personally. I should've been well enough to stand up and declare your authority. But I wasn't. And it's all my fault.
I'm sorry, kid. I really am. I saw dollar signs, and in the end I screwed us both.
