The Fallout world is not our own. - The USS Ebon Atoll is accidentally sunk by the USS Interference during the Anchorage Campaign, killing all on board. It is one of the worst U.S. Navy disasters since World War II.[155]

- August: Around this time, Caesar's malignant brain tumor begins causing mild headaches, only growing worse with every day and soon starts affecting his motor functions and coordination. He is unaware of the cause and has few reliable means to diagnose, much less treat it.[711]
- August: Beagle becomes the deputy of Primm thanks to his sister marrying the incumbent sheriff.[712]
- September: Cassidy Caravans is raided by an unknown group of assailants for unknown reasons. The owner Rose of Sharon Cassidy was not with her caravan at the time, but word reached her while she is stuck at the Mojave Outpost.[713]
- September: The NCRCF prison break occurred around this time. Prisoners incarcerated at the NCR Correctional Facility used a supply of dynamite to kill their guards and seize control of the prison. Afterwards, the convicts split up into separate groups: one led by Samuel Cooke went north and settled in Vault 19. Another went south and attacked the town of Primm, taking over the Bison Steve Hotel[714]. Eddie reorganized the rest into keeping control of the NCRCF, calling themselves the Powder Gangers.
- September: Frumentarius Karl goes on a mission to Red Rock Canyon to broker a deal between the Great Khans and Caesar's Legion. He manipulated the Khans, leaving them unaware of what joining the Legion would mean for their tribe while stoking their anti-NCR vendetta.[715]
- Carla Boone is abducted from Novac and sold to Caesar's Legion along with her unborn child.[716] Her husband Craig Boone tracks her down to the Legion port at Cottonwood Cove. Unable to rescue her, he makes the decision to kill her with a headshot to spare her living horribly as a slave.[717][718][719]
- Raul Tejada travels to Black Mountain to investigate the sudden lack of broadcasts coming from Black Mountain Radio where he's captured by Tabitha and the State of Utobitha, but is spared when he offers his mechanical expertise to fix the radio technology. He is imprisoned and forced to perform repairs for the community on frequent threats of death.[720][721]
- A pack of Deathclaws overrun Quarry Junction after the Powder Gangers steal Sloan's dynamite supply. Mining operations halt and Interstate 15 is shut down due to their presence.[722]
- A scavenger team working for House finally recovers the intact platinum chip in Sunnyvale. House organizes a delivery, using decoy couriers and spending tens of thousands of caps on mercenary screenings to ensure its safe passage to New Vegas.[723] 5 of the 6 are dummy packages of novelty junk, but one Courier is given the platinum chip with orders to deliver the package to the The Strip North Gate by way of Freeside.[724][705][725]
- October 5: Knight Christine Royce, sent by the Circle of Steel to kill Father Elijah, is trapped in an Auto-Doc in the Sierra Madre Villa clinic. She remains trapped for two weeks, suffering a looping medical procedure until the Courier arrives.[726]
- October: Courier Six takes a job from the Mojave Express and is elected the one to carry the platinum chip after another courier backed out. The Courier travels from southern NCR, through the Hub to reach the Mojave,[Non-game 182] and makes their way to the Goodsprings area, unaware they were walking into a trap.
- October 8–11: With the help of the modified Securiton, Yes Man, the rogue leader of the Chairmen, Benny plans to intercept the Courier's delivery, steal the chip and use it for his own ends. He hires Jessup, McMurphy, Chance and several other Great Khans to assist him in the ambush. During the on-foot trek, however, Chance loses his life to wounds sustained during an attack by the Fiends and a fatal drug overdose secretly orchestrated by Benny.[727]
- October 11: Fallout: New Vegas intro - The Courier is captured, robbed, then shot twice in the head by Benny. After being left for dead in a shallow unmarked grave in the Goodsprings Cemetery, Victor retrieves them, delivering them to Doc Mitchell for medical care, though they remain unconscious for 8 days.[728][729]
- October 12: Joe Cobb's raiding party attacks a group of Crimson Caravan traders on the Long 15. One survivor, Ringo escapes and flees to Goodsprings, pursued by the Powder Gangers.[730]

- New California Republic Senate elections are due soon. Representatives from the Boneyard block further military funding to gain votes from voters dissatisfied with the lack of progress in the Mojave Campaign.[735]
- The Courier is lured to the Sierra Madre through the abandoned BoS bunker. Captured by Dog, they awake in the Villa, and are forced by rogue Elder Elijah to assemble a team and carry out a heist targeting the casino's vault. The Courier outsmarts Elijah and escapes to the Mojave, letting the Sierra Madre go.[736]
- The Happy Trails Caravan Company organizes a caravan to Zion, under Jed Masterson, to link up with New Canaanites there. The caravan is ambushed, leaving the Courier as the sole survivor. They join Joshua Graham and Daniel, overcoming the White Legs in the conflict with the Sorrows and Dead Horses.[737]
- A satellite crashes in the Mojave Drive-in. The Courier once more investigates, and is transported to Big Mountain. They are lobotomized and reduced to the state of a walking cadaver briefly, until they meet the Think Tank and are embroiled in a decades-old conflict with Mobius. They ultimately prevail, returning to the Mojave with spoils of Science![738]
- Ulysses draws the Courier onto the Lonesome Road. The two couriers confront each other at the edge of the world, beneath an Old World flag, and decide the fate of the Legion and the New California Republic.[739]

- Rachel Watkins leaves Kiddie Kingdom to find a cure for feral ghouls. Oswald the Outrageous remains behind to defend Kiddie Kingdom.[758][759] Her work fails when she starts going feral and commits suicide to protect others.[760]
- Winter: After performing experiments on his followers for several weeks, the Last Son of Atom launches a holy war against the settlements of the Commonwealth, destroying several communities.
- Late Winter: Mechminster Abbey is destroyed by the forces of the Church. The Last Son is either killed or driven west. The rolling city of Mirage leaves the Commonwealth.

- Summer: Colter establishes the Gauntlet to sate the raiders' bloodthirst when he fails to annex the rest of Nuka-World.[755]
- Fall: The Gauntlet runs its course and the raiders again become dissatisfied with Colter's leadership. Gage secretly forms an alliance with the heads of the three raider clans, preparing a trap for Colter. Nisha provides the weapon to disable Colter's power armor, while Gage arranges a ruse to attract their unwitting assassin.[761][762]
- Recon Squad Gladius, the third of the Brotherhood's scout teams and led by Paladin Danse, arrives in the Commonwealth, setting up base within the Cambridge Police Station. Due to a string of bad luck, four of the seven members are killed within weeks of arrival.[702]
- The Institute carries out a series of attacks against safehouses operated by the Railroad, including purging their headquarters at the Switchboard. Once again, the Railroad is nearly wiped out.[763]
- Theodore Croup is killed in the basement of his family's home by the feral, ghoulified remnants of his family after two hundred years of attempting to help them regain their humanity.[764]
- The Quincy Massacre takes place as Gunners rout the small group of Minutemen defending the city and massacre most of the town's population. The survivors flee into the wasteland with Preston Garvey, losing most of their number in the long retreat to Concord.[765]
- October 23: 9:00 a.m. Fallout 4 begins – Two hundred and ten years after the Great War, the Sole Survivor awakens from cryogenic suspension, leaving Vault 111 in search of their son, Shaun.[766]

- After decoding the chip with the help of the Railroad, the Sole Survivor enlists the help of one of the three major factions to construct a signal interceptor and hijack the Institute's teleporter to enter themselves. They meet their son Shaun, who is now an old man and the Director of the Institute.[772]
- The Sole Survivor becomes a trump card in the war between the Brotherhood, Institute, Railroad, and Minutemen, and ultimately resolves the War of the Commonwealth for one of the four factions.[773]
- The Sole Survivor helps the robot Ada avenge the deaths of her human friends and put a stop to the robot threat orchestrated by the Mechanist.[774]

- The Brotherhood of Steel hears about Dr. Wilzig's escape and commences a mission in search of Dr. Wilzig and his technology. Other factions also hear of this event, and a large bounty is set on Dr. Wilzig.
- Aspirant Maximus joins the search operation of Dr. Wilzig as a squire under the command of Knight Titus. The Ghoul, a bounty hunter, also starts to hunt him down in order to claim the bounty.

- Following the kidnapping of Hank MacLean and Vault 33's recovery from the raider attack the previous year, Betty Pearson is elected as the Vault's new overseer. She initiates a reassignment program, moving many of Vault 33's dwellers to Vault 32 to repopulate the destroyed Vault, with Steph Harper declared as its overseer.
- Lucy successfully brings Dr. Wilzig's head containing his cold fusion technology to the New California Republic headquarters at the Griffith Observatory, intending to exchange it for her father's freedom. Instead, she learns from Moldaver the truth behind her father's identity, her mother's death, and the destruction of Shady Sands in the process.
- Norman MacLean discovers the truth about Vault 31 and is locked inside by its overseer, pre-War Vault-Tec executive Bud Askins.
- The Brotherhood occupy the town of Filly, and later attack and defeat the New California Republic remnants led by Moldaver at the Griffith Observatory, seizing control of the observatory and the cold fusion technology.
