Detailed walkthrough[]
The next stage of Mr. House's preparation for Hoover Dam involves investigating one of the Strip's landmark casinos, Gomorrah, run by the Omertas of the Three Families. He tells to speak to the receptionist to gain more information.
The casino lies directly across the street from the Lucky 38 on the Strip. Like all casinos, upon walking through the doors, one will be required to disarm. Refusing will lead to the Omertas turning hostile and must be killed, while agreeing and then talking to the receptionist will trigger How Little We Know, which runs concurrently with this quest. Upon completing that quest or wiping out the Omertas, the player character must then return to Mr. House to report the outcome, completing this quest and triggering The House Always Wins V.
If one has previously completed How Little We Know, the quest will mark as completed automatically and House will reward the player character anyway just to avoid discouraging them. Alternatively, if one refuses to disarm upon entering the casino or otherwise acts in a way to turn the Omertas hostile, there is no need to actually kill any of them; one can turn them hostile, then quickly leave and go back to speak to House. This will fail the quest but still trigger The House Always Wins V; this is the fastest way to progress through this quest but forfeits any rewards.
Note: If one completed How Little We Know by siding with Nero and Big Sal, the quest will automatically fail upon returning to speak to Mr. House, causing him to turn hostile unless one can pass a 50 Speech skill check to convince House that they had secretly infiltrated the Omertas in order to gain their confidence and learn about their plans, allowing one to continue supporting House and trigger The House Always Wins V.

Completing this quest will automatically fail Don't Tread on the Bear! and Beware the Wrath of Caesar!, permanently locking out the NCR and Legion endings.
Bugs[]
- set "116b58".OmertaQuest to 0. This will treat it as if the Omertas were already dealt with before receiving the quest from Mr. House. Without having a save point prior to killing Big Sal and Nero, the quest can be fixed by using the console command
After eliminating the Omertas, Mr. House doesn't recognize that they are gone. To remedy this, load a save point before killing Big Sal and Nero, go talk to Mr. House and make sure you go through all the dialogue on what to do about the Omertas. After this, killing Big Sal and Nero and returning to Mr. House should allow you to finish the quest. [verified]
- When working on Part IV of the mission, there will be an option while talking to the Omerta receptionist that says [Barter x/55], meaning you need at least 55 Barter to choose it. But upon using a Salesman Weekly to raise Barter to 55, the option goes away, and you are left with the [Strength x/8] option or the option to hand over 300 caps. Leveling up the Barter skill does not help. [verified]
