Surgical waiver is a paper note in Fallout 76, introduced in the Burning Springs update.

This note can be found on a desk next to a gurney in the Clinic in Highway Town. The Clinic is above Clark's chem shop on the main level.

This Highway Town sanctioned practice assumes no liability for injury, loss of life, or complications arising from examinations or surgeries. All patients acknowledge that they proceed at their own risk.
Furthermore, they fully understand & accept that Dr. Lacey Brown was originally trained as (a veterinary surgeon, not) a human physician, and consent to treatment with this knowledge.
In the unlikely event of death or dismemberment, all remains become the sole property of Dr. Lacy Brown.
Post-inspection or post-surgical side effects are the sole responsibility of the patient. Any request for additional work or examinations will be considered a new and separate payment. This includes, but is not limited to, perpetually sneezing, stomach ache, loss of cognitive function, or death.
