The skull and its various fragments are of human origin. They may be found scattered throughout Appalachia but, with a few exceptions, cannot be removed from complete skeletons.

- Five in a hot tub at the cabin south of North Kanawha lookout.
- Found scattered on the ground in ritual mounds east and west outside of Vault 76.
- One can be found held by a skeleton at the unmarked bog channel shelter north of Quarry X3.
