This title by is obtained by having sex ten times.
From a technical perspective, the counter is increased whenever a dialogue calls the do_default_sex procedure (see COMMAND.H), which increments the GVAR_SEX_COUNTER global variable and grants the Sexpert title on the tenth intercourse. If a game doesn't call it, then the variable will not be incremented (such as when purchasing services at Cat's Paw).

Improves the player character's sexual dialogue.
It's one of the perks factoring into the game's sex rating.
