Out of order is a paper note that was cut from Fallout 76, introduced in the Expeditions: Atlantic City update part one, Boardwalk Paradise.
The note is supposed to appear at the Neapolitan Casino during the Expedition quest Tax Evasion, but is only placed in a test cell and cannot be acquired in-game.
