An ophthalmoscope is a medical instrument used to examine the eyes. However, the object in the game resembles an otoscope, which is used to examine the ears.
The ophthalmoscope can be recognized as the medical instrument James uses to observe the Lone Wanderer's eyes before the G.O.A.T. exam. However, no light appears to be emitted from the ophthalmoscope, which is necessary to effectively examine the eyes.
This item can be used as Rock-It Launcher ammunition.

Olney Powerworks - Go where the Tesla coil is located, but instead of taking it, go inside the passage door near the Tesla coil.- Old Olney S. Wilson Building, in the northeast, in a bucket.
- Two in the mobile base crawler, one on a shelf, the other near a turbine.
- One in the satellite control tower, on a shelf in the northeast.
- Adams Air Force Base, south hangar on a table.
- The morgue in Turtledove Detention Camp.
- Sandra's office has one on the medical cart beneath the first aid box.
- On a curved table in the abandoned apartments, near the Uptown exit.
