The note can be found pinned to the wall of a cell in the catacombs of the Sanctum in The Pitt during From Ashes to Fire. It is near the outstretched hand of a laborer's corpse.

We tried! We did everything we could to get her out but we just couldn't move any more of the blocks. All I could do is hold her hand until the end.
Her name was Ness Myers.
