The pre-War United States was comprised of 13 commonwealths, acting as an intermediate level of governance between the state and federal governments. Intended to help create legislation broad enough to benefit states with common regional concerns, but narrow enough not to affect those with dissimilar interests or political cultures. Following the Great War, the commonwealths disintegrated into what became known as the wasteland. When land across the country eventually became less radiated and more habitable, survivors reestablished settlements and began new post-war lives. The commonwealths include:

Columbia Commonwealth - Comprised of the former states of Maryland and Virginia, as well as the District of Columbia.
- Capital Wasteland - Spans much of the District of Columbia, as well as portions of both Maryland to the north and Virginia to the south. The area serves as the game-world for Fallout 3.
- Point Lookout - A swampland that was once a state park in the former state of Maryland. The area serves as the game-world for the Fallout 3 add-on Point Lookout.
East Central Commonwealth - Comprised of the former states of Ohio, Kentucky, and Tennessee.
Eastern Commonwealth - Comprised of the former states of West Virginia, Delaware, Pennsylvania, New Jersey, and New York.
- The Pitt - A major raider city-state built on the remains of Pittsburgh, Pennsylvania. The area serves as the game-world for the Fallout 3 add-on The Pitt.
- Appalachia - A vast swathe of the former state of West Virginia, which was also known pre-War as the Appalachian Territories. It serves as the game-world of Fallout 76.
Four States Commonwealth - Includes the former states of Utah, Colorado, Arizona, and New Mexico.
- Zion Canyon - A state park in the former state of Utah. The area serves as the game-world for the Fallout: New Vegas add-on Honest Hearts.
Great Midwest Commonwealth - Comprised of the former states of Wisconsin, Minnesota, Illinois, Indiana, and Michigan.
Gulf Commonwealth - Includes the former states of Louisiana, Mississippi, Alabama, and Florida.
New England Commonwealth - Comprised of the former states of Maine, New Hampshire, Vermont, Massachusetts, Rhode Island, and Connecticut.
- The Commonwealth - Refers to a portion of the New England Commonwealth, the former state of Massachusettes. It serves as the game-world for Fallout 4.
- Far Harbor - A landmass on the former state of Maine's coast. The area serves as the game-world for the Fallout 4 add-on Far Harbor.
Northern Commonwealth - An area comprised of the former states of Montana, Wyoming, North Dakota, and South Dakota.
Northwest Commonwealth - Comprised of the former states of Northern California, Washington, Oregon, Idaho, and Alaska.
Plains Commonwealth - An area comprised of the former states of Nebraska, Kansas, Iowa, Missouri, and Oklahoma.
Southeast Commonwealth - Comprised of the former states of Georgia, South Carolina, and North Carolina.
Southwest Commonwealth - An area comprised of the former states of Southern California, Nevada, and Hawaii.
- New California - A loosely defined area of the former states of California, Nevada, and parts of Mexico. The area serves as the game-world for Fallout and Fallout 2.
- Mojave Wasteland - The area is synonymous with the old world Mojave Desert, spanning large portions of the former Southwest United States. The area serves as the game-world for Fallout: New Vegas.
Texas Commonwealth - Comprised of the former states of Texas and Arkansas.

Vaults

The Vaults are a series of survival shelters designed by Vault-Tec Corporation on commission from the U.S. government to protect a selected portion of the United States population from a nuclear holocaust. The government commissioned 122 Vaults as part of Project Safehouse, seen throughout the continental United States.
