The note can be found on the patio of the tavern at the Pleasant Valley cabins, on a dining table, only during Seeker of Mysteries.

This note can be missed, as the quest marker for it disappears after looting Tony's orders. The note will still remain available while the quest is active, but will de-spawn after the quest is completed and cannot be obtained afterward.
