Lead Belly is an Endurance perk in Fallout 4.

Taking ranks of this perk reduces Radiation intake from eating non-cooked food (e.g. raw meat, Sugar Bombs, Salisbury Steak, etc.) or drinking non-purified water. Higher ranks reduce the damage further, up to being completely negated.

- Ranks 1 and 2 of this perk allow the non-linear Radiation Resistance function, which works similarly to Damage Resistance, to be applied to reduce the rads received from ingested food and drink. For example, a Radiation Resistance of 10 (rank 1) will reduce the rads in an item that normally delivers 10 rads by half, to 5 rads. A resistance of 20 (rank 2) will reduce the rads in an item that normally delivers 20 rads (only possible by drinking irradiated blood) by half, to 10 rads. The reduced rad effect of consuming the food or drink item is shown in the Pip-Boy, rounded to the nearest whole number: a stated rad count of 0 indicates a fractional value that is below 0.5 rads but likely above 0 rads.
- Taking this perk early on may help out a lot, as most available food (raw or pre-War) carries significant rads, and RadAway is scarce in the beginning. Conversely, this perk has little value in the late-game stages because foods provide only a fixed amount of Hit Points (whereas stimpaks restore a percentage). However, the player gains access to a cooking station very early on, so provided they can fast travel, the player character should rarely be eating raw food.
- Playing on Survival mode, however, is where this perk becomes more useful all throughout early to late-game due to the necessity of regularly eating and drinking to stay Satiated and Hydrated.
