The note can be found at the Pleasant Valley cabins, behind the tavern near a Port-A-Diner, only during the quest Seeker of Mysteries.

This note can be missed, as the quest marker for it disappears after looting Tony's orders. The note will still remain available while the quest is active, but will de-spawn after the quest is completed and cannot be obtained afterward.
