Ghoulish is a perk in Fallout 76.
It was also available as a perk in the defunct Nuclear Winter battle royale mode.

- Adventure
- Ghoulish 1 / 2 / 3 restores 0.5 / 1.0 / 1.5 HP for every 1 radiation damage acquired, this only works while actively being irradiated. Ghoulish will not trigger if the player's radiation level does not increase.
- Synergizes extremely well with the perk What Rads? to counter the negative effects of being irradiated. It also works well with the mutations Healing Factor.
- Nuclear Winter
- Ghoulish restores 1.25 HP for every 1 radiation damage acquired.
- Radiation resistance through Power Armor, Rad-X, etc will decrease the effectiveness of Ghoulish.
- Ghoulish does not eliminate radiation damage. As radiation accumulates, available HP capacity will continue to decrease.
- Conversion note: HP and radiation damage are not converted at a 1:1 ratio. It takes 5 radiation damage to consume 1 HP. In other words, with Ghoulish 2 in Adventure, 1 radiation damage would restore 5 HP.
