A settler must be assigned to the clinic to function as a store and to raise settlement happiness.
The first aid station is the first tier store, the outdoor stand is constructed from wood with a crudely drawn white cross fixed to the roof and the word "chems" hanging from the crossbar. The second version is the store counter which has the word clinic written across the front in red capitals.
The clinic is the second tier store. The outdoor stand with "clinic" written on a sign affixed to the roof and the other with word "chems" hanging from the ceiling on the right hand side. There is also a white cross on a red background painted on the front of the counter. The store counter variant is identical in appearance to that of the first aid station.
The surgery center is the top tier store. The outdoor store is constructed from metal, with front shutters that have a picture of a white cross painted on and the word "clinic" written underneath, which is affixed to the shop roof. The store counter has the word clinic written across the front left hand side in red capitals with two rows of backless shelves on the right hand side, consisting of 2 shelves to a row. The surgery center has the widest range of goods available from the three medical stores.
All three tiers can be used in the same way as an NPC doctor in a settlement. They can treat health/rads/chem addictions, and in Survival mode, can cure illnesses.

- Doc Anderson can be assigned to the surgery center to upgrade it to a level 4 medical shop.
- In the add-on Far Harbor, clinics of all tiers located on the Island provide only the Barter functionality (mostly Chems and other Aid). They do not provide any medical services, i.e. healing hit points, disease, addiction or radiation. These services are only available from NPC doctors. It is not clear if this is a bug or a design decision.
