The note is given to the player character automatically after interacting with the piano in Concerta Lombardi's suite, room #105, in the hotel wing of the Neapolitan Casino, during the quest Regent of the Dead.

During the first Public Test Server for the Boardwalk Paradise update, this note was datamined from the game's files, previously being titled Note to self, though it was not able to be acquired in-game. Its previous transcript read:
Need to move Q into the old chamber behind the VIP room. He can't stay where he is now.
The note was reworked and retitled to its current version with the first Public Test Server for America's Playground.
