Would be hard due to the format Fallout has done. I can picture the Synth/Supermutant could have twist moments of the protagonist the player is a Sunth the whole time. Or the player is actually a modified supermutant that managed to maintain the humanoid aspect. An actual super human/soldier

Ghouls or super mutants would be interesting. A gen 3 synth is no functionally different from a human.

Ghoul would be cool, and they wouldn’t have to change that many models

I think practically speaking a ghoul is the most likely to happen as they design all the outfits around fitting the human body so they could very easily accommodate a ghoul option.
