Oh, I know what you mean. I have mods that let you save easier. Completely negates that problem.
I think it could definitely work, Dead Money has some survival horror aspects and it's one of the best pieces of Fallout content ever made.

I haven't (lmao), but I've heard it's kind of a hardcore new vegas horror conversion. You play in a timeline where absolutely everything has gone the worst possible way it could go and the end goal is leaving the desolate mojave. Sounded pretty good at least.
Yeah, I've played it. Its not that hard, atleast to me. I find the spiritual successor to it Fallout 4 FROST harder. DUST is pretty easy after about an hour. Just use stealth and be super alert of your surroundings and it gets easy. The only things that have me trouble was the Skin walker Joshua Graham, and all the fucking Ghouls in freeside and the strip. I was kinda let down by the amount of ghouls. The mod made it out to be like a massive walking dead horde (like below) but it's only like 20 or so ghouls placed kinda close together. The tunnelers was never an issue for me, even in the base games dlc. I'm the kind of person who can easily kill entire nests of cazadores without dying. The only enemy that has consistently been a pain in my ass in the series are fucking bloat flies. I swear to god those bastards have hidden code that makes you miss 99 percent of the time, or makes your bullets go right through them.
I guess they didn't have much faith in gamebryo, the only time I've seen a mod push the number of enemies as high as it could was on new cali on those missions where enemies are just endlessly spawning until the objective is completed.
Play the Better Angels by Some guy 2000. It's basically WW1 trench warfare against the legion. There's one point where about 200 NPCs are fighting at once. Basically a large legion slave revolt happened, and the NCR is trying to help them get across the river. You have to lead about 100 slaves across the river. Once over there, Im not lying, about 800-1000 Legion member's attack the NCR encampment. It's you and 100 NCR troops and rangers and the slaves against a legion onslaught. It's awesome and a fuckin hell of a way to end a mod series. It's the final part of the Someguy series.
I think they could if they made it a very small game(like 10 hours of gameplay) to focus on a dark and grimey setting or situation. I would like to think of it like Metro Lost Light(when in the haunted areas) and Metro(dark tunnels) where it has simplistic gameplay but the quality of the simplistic studd are done well.
Here how I can picture it. Your are a Brotherhood of Steel Initiate or whatever rank. Knight lets say. And your whole squad is dead and you are lsot with high radiation, damaged armor, limited bullets and equipment and you need to get out of a infested place filled with [Insert whatever enemy]. You need to get out, find the dog tags of your lost squadmates, managed health and weapon condition. But that would be my idea of a Fallout Horror survival. Which might be a Horror FPS at that point and not a survival.
^ Your BOS sqaud go to investigate a vault. You're an initiate, this is your first combat deployment. You're Not given Power Armor. You get in, the vault door shuts behind you. It's pitch black, save a few lights from the dimily lit lights that barely work. Suddenly out of nowhere some creature you don't get a good look at attacks your sqaud. Your knocked unconscious. You wake up and find a squad mate dead, and the other 2 missing. You take the dead squad mate's pistol ammo, and venture forth into the vault looking for your missing sqaud mates and a way out. Inside a rogue Geck with an FEV sample has created awful mutations and parasites and turned the dwellers and those unfortunate enough to stumble in here before you. Think Dead Space, but it's in a vault. No pipboy, you use a backpack with limited storage space, and a flashlight for light. Ammo and supplies are extremely scarce. A gasmask you have to wear in some areas because of toxic air, Radioactive air, and smoke clouds.
