He is just a human having a midlife crisis and convinced he's a ghoul. Not actually a ghoul, as the actual ghouls there will tell you. Even they couldn't talk him out of his delusions, so they just went along with it.

Depending on what part of you houses the mutation, the body or the brain, hypothetically the Big Empty machine could be used to remove the Brain, and using the same techniques the Think Tank had planned on using, placing the brain into a new host body. But that’s dependent on the mutation not being able to spread from a transplanted brain to the new body.

There's no known cure for the ghoul condition and the only known treatments are prophylactic; I.e. don't expose yourself to radiation because becoming a ghoul is the best thing that you can hope for. Whilst it's not confirmed, it's reasonable to assume that ghouls are resistant to radiation but not totally immune to prolonged exposure to high radiation levels.

My personal theory is that feral ghouls are the result of ghoul's biological immortality, a lot of them appear emaciated as if they would have starved to death otherwise. There's anecdotal evidence that degraded conditions and in particular radiation correlate with high proportions of ghouls turning feral. Nuka-World's kiddie kingdom in particular would seem to indicate that sane ghouls are adversely affected by radiation exposure in the long run, that's a theory I'm fairly confident in. I'm not as confident in extrapolating that to other health concerns, there's either insufficient or inconsistent information in that regard. It's a solid enough theory that I'd be willing to work on the assumption that ghouls require basic standards of welfare given the lack of any real downside and the fact that it's just the right thing to do anyway.

You are talking about two Ghoul conditions. For the first, it is unexplained why a face swapper can alter a person's appearance radically yet would find it impossible to give Bobbi, Eddie Winter or Hancock a normal appearing face. It is not plausible that you can do one but not the other, and yet it must be impossible because clearly there was great motivation of the former DC residents to get face/ body major cosmetic surgery. Somehow such procedures dont work on ghoul physiology. Maybe they could in principle work, but practioners only have tools, drugs and grafting techniques that work only on normal human tissue.

Feralism is different. The cure that Oswald wants would restore a feral ghoul to their senses. The lore is that feralism involves physical destruction of the brain. Given that stimpacks can restore massive physical damage, it is plausible that brain physical structures could be rebuilt. Stimpacks do work on Hancock. However restoring a physical brain would not restore the non physical mind. That stuff would be lost. The mental condition would be something like that of a baby- which actually is what would realistically be the result of a stimpack healing Hancock after a gunner blew most of his skull away.

But whatever. As Virgil says, don't ask me how it works. It just does. If the molecular relay really reassembles molecules at destination, then it is a duplicator and Father could just replicate himself from a stored version of his younger version of himself prior to his medical condition.

In Oswalds case he is referring to his love, a fellow pre-war ghoul named Rachel. She was an organic chemist who couldn't stand the thought of her friends, the ghouls in Kiddie kingdom, going feral from their affliction. Unable to find a 'cure' she went out into the world in hopes of finding some way to help her friends and any one else similarly afflicted. Unfortunately, she did not. You can find her, and a holotape meant for Oswald, in a wrecked house at the edge of Bradberton town, just outside of the Nuka World park.

As for a 'cure' in general for ghoulism, the reversal of radiation damage to the body and the reversal of damage to the brain causing the degradation into the feral state, I doubt that there is. Possibilities exist, sciences that could be adapted, maybe, But a full, 'cure'?, I would have to say no, IMO.

As to why some go feral and some do not, don't know. Why do some people get cancer after years of smoking and some do not? Some tweak in the bodies immunity system is better that others..., and or some people, or ghouls, just get lucky like that.
